Educational quality of laparoscopic hysterectomy videos for advanced endometriosis: institutional archives versus YouTube and MedTube

Objective: This study aimed to evaluate the educational quality of laparoscopic hysterectomy videos for advanced endometriosis (ASRM stage III − IV) and to compare them across different digital platforms. Methods: In this observational comparative study, laparoscopic hysterectomy videos were selected from YouTube, MedTube, and our institutional video archive during the period from September to November 2025. Videos were assessed using the nine-item LAParoscopic surgery Video Educational GuidelineS (LAP-VEGaS) scoring system. Each video was independently evaluated by six reviewers, including three senior surgeons and three residents. Data were analyzed using the Kruskal–Wallis test and linear mixed-effects models. Results: A total of 60 laparoscopic hysterectomy videos were analyzed, with 20 videos from each platform. Median LAP-VEGaS scores were significantly higher for institutional archive videos [14.0 (13.0–15.0)] compared with YouTube and MedTube (p < 0.001). Multivariate analysis using a linear mixed-effects model showed that the institutional platform (β = +2.13, p < 0.001) and senior surgeon evaluators (β = +0.28, p = 0.038) were independently associated with higher educational quality. MedTube (β = −0.22, p = 0.676) and endometriosis stage IV (β = −0.81, p = 0.101) were not significantly associated with LAP-VEGaS scores. Videos from the institutional archive achieved the highest scores, particularly for Educational Value (Criterion 9) and Image Quality (Criterion 8). Conclusion: The educational quality of laparoscopic hysterectomy videos is more closely related to academic and institutional context than platform type. Structured, academically curated video archives may play an important role in supporting standardized surgical training for complex procedures.
